Instructions for disability and FMLA papers

Delaware Valley OBGYN & Infertility Group understands the need for patients to have Disability and FMLA forms completed.  If you need Disability / FMLA forms filled out for any time off from work due to pregnancy or surgery - please make sure you fill out all of your paperwork in the sections marked 'Employee' and sign all employee signature areas before bringing them to the office.  In addition to the forms provided by your employer, you will also need to print and fill out our Disability Forms Request.

Please allow up to 7 to 10 business days for forms to be completed and there is a $10 form fee (per set of forms) that will need to be paid before the forms are picked up. No exceptions will be made.  If you need them faxed back - the fee is an additional $2 for total form fee of $12.

You can fax your forms to the office at (609) 896-3266, email to Violet vchambers@LMA-LLC.com or simply drop them off during normal business hours.

We take no responsibility for any forms lost in the mail. Please note that it is the patient’s responsibility to provide the completed forms to their insurance company or employers.  Please do not hand these forms to our physicians or nurses, as they see numerous patients a day and forms can be misplaced.
